Data Usage Request Procedure and List of Data Stewards

This article explains the procedure for data usage requests.

The Data Stewardship Council has produced a standard form to coordinate the request for data from various university sources. The advantages of having a standard form include:
  • Consistency of requests for ease of use for data stewards
  • Documentation for audit and approvals

The Data Usage Request (DUR) form is available as an attachment here: DUR Print Form. Once it is completed, it should be printed and then signed by the requestor. The requestor can then route the DUR to each Data Steward as needed. IT Services may assist requesting departments in understanding specific data, source and technical issues, however it is the requesting department's responsibility to initiate and submit the DUR to the Data Stewards.

A Data Steward may approve some but not all of the fields or type of data requested. For example, a requestor may get permission to receive business but not home addresses, or to get data for undergraduate students, but not for graduate students.

This email list should be copied on all emails to the data stewards: DataUsageRequest@lists.uchicago.edu.

This list may also be used for support and advice from the Data Stewardship Network.

Photo Files: Photos must be requested as a separate data element. They are considered highly sensitive data. Faculty and others may opt-out of sharing their photos.

Note: Postdocs require approval from both Staff and Faculty Data Stewards.
